ATTENTION ALL PEBTF PARTICIPANTS:
Please remember that you must complete a biometric screening plus complete the annual health risk assessment to continue to qualify for the “Get Healthy” waiver.
You can have the biometric screening done in any one of three ways:
Register online and attend one of the onsite events;
Register online and have it done at one of the Quest Patient Service Centers;
Obtain a form and have your doctor fill it out and fax it in.
The last onsite event will occur on January 22, 2014. If you choose to have your screening completed at your doctor’s office, you must have it completed and have your doctor fax the results to Quest no later than January 10th. If you choose to have the screening done at a Quest location, it must be completed no later than January 31, 2014.
