Important Information About the PEBTF’s Get Healthy Program and the
Contribution Waiver Requirement for July 1, 2014
Just to clarify the highlighted section the H1 Bargaining Unit is at 3%.We will negotiate the contribution in this upcoming Interest Arbitration.
The PEBTF sent this email out to all State Employees who are at 5%.
Completion of the Know Your Numbers wellness screening is the only requirement this year. The Get Healthy health assessment is not required.
You will earn the health care contribution waiver for the period July 1, 2014 to June 30, 2015 by completing a Know Your Numbers wellness screening by January 31, 2014. You can still obtain a wellness screening at a Quest Diagnostics Patient Service Center. If you do not complete a wellness screening in time, your health care contribution will increase to 5% of your gross base salary in July 2014!
To register for a wellness screening, call Quest Diagnostics at 866.908.9440 (available Monday – Friday, 8 a.m. – 9:30 p.m. EST and Saturday 8:30 a.m. – 5 p.m. EST).
You and your covered spouse/domestic partner do not need to complete a Health Assessment survey in the spring of 2014.
The Get Healthy Program requirements are subject to change in the future and this change is effective for this year only. Any changes will be communicated to you via mail and email in advance of the change.
